Key Responsibilities\n- \n
- Conduct thorough psychosocial assessments of patients through structured interviews, direct observations, and detailed medical record reviews to identify emotional, social, and environmental factors that may influence their recovery journey. \n
- Provide empathetic counseling and support to patients and their families, helping them navigate the emotional challenges associated with end-of-life issues and adjustment to disability or illness. \n
- Collaborate proactively with the broader rehabilitation team—comprising physical therapists, occupational therapists, and medical professionals—to develop tailored, goal-oriented treatment plans that address individual psychosocial needs. \n
- Assist patients in identifying and accessing essential community resources such as support groups, vocational rehabilitation programs, and housing assistance to promote a seamless transition home or to alternate care settings. \n
- Facilitate discharge planning to ensure smooth transitions that support independence and continued care, which includes educating patients and families about community support services post-rehabilitation. \n
You will be working closely with a diverse patient population at our hospice center. This includes individuals experiencing chronic illnesses, terminal diagnoses, and major life transitions. Your approach will account for varying emotional responses, cultural backgrounds, and family dynamics as you prioritize holistic care that encompasses their psychological, social, and emotional well-being.

- A Master’s degree in Social Work from an accredited program is mandatory. \n
- Current state licensure as a Licensed Clinical Social Worker (LCSW) is required to practice effectively. \n
