Work Environment

The Physical Therapist will operate in our well-equipped outpatient office located in beautiful Yonkers, New York. The facility is designed to provide a patient-friendly atmosphere, with dedicated spaces for daily therapy sessions and rehabilitation activities. As this role is onsite, you will interact directly with residents, caregivers, and a multidisciplinary team daily, fostering a supportive and engaging therapeutic environment.

Key Responsibilities

In this role, you will have an active role in patient recovery and rehabilitation efforts. Your key responsibilities will include:

• Conducting comprehensive physical evaluations to assess each resident's range of motion, strength, balance, gait patterns, and functional limitations, ensuring no detail is overlooked.
• Developing individualized treatment plans based on assessment findings, establishing achievable goals, and selecting appropriate modalities, including exercises, heat therapy, or manual therapy.
• Implementing treatment plans with activities explicitly designed to enhance strength, flexibility, balance, and coordination, while actively addressing and managing pain.
• Assisting residents with proper walking techniques, utilizing assistive devices, and instructing on fall prevention strategies to promote safety and mobility.
• Focusing on Activities of Daily Living (ADLs) such as dressing, bathing, and transferring to ensure residents can maintain independence or require minimal assistance.
• Educating residents and their caregivers on correct exercise techniques, home exercise regimens, and proactive fall prevention to extend recovery benefits beyond therapy sessions.
• Maintaining comprehensive patient records, including timely progress notes, treatment plans, and functional assessments, to ensure continuity of care and facilitate communication with team members.

Patient Population / Client Focus

This position primarily focuses on elderly individuals residing in assisted living facilities. Your expertise will be critical in addressing age-related conditions such as arthritis, osteoporosis, dementia, and Parkinson's disease. You will play a vital role in enhancing the physical health and overall well-being of these residents, aiding their recovery and functional independence.

Qualifications

To be considered for this role, candidates must meet the following qualifications:

• A valid Physical Therapy license in the state of New York is required.
• A Bachelor's Degree in Physical Therapy from an accredited institution is necessary; a Master's or Doctorate is preferred.
• Strong communication and interpersonal skills to foster effective relationships with patients and caregivers.
• Proficiency in developing and implementing patient-specific treatment plans tailored to individual needs and conditions.

Preferred Experience

Compensation and Benefits
