Key Responsibilities\n

• Conduct comprehensive assessments to determine patients' needs, utilizing evidence-based approaches.

• Participate actively in Interdisciplinary Team Meetings and contribute valuable insights relevant to ongoing patient care.

• Document monthly and quarterly progress notes to track and communicate patient advancements.

• Organize training sessions to ensure team members are proficient in executing program plans and maintaining accurate documentation.

• Conduct follow-up interviews and additional home visits as necessary to provide continued support for each patient's evolving needs.
