Key Responsibilities\n

• Conduct thorough and continuous assessments of patients to tailor treatment plans that address their specific needs and concerns.

• Facilitate communication with supervisors, healthcare team members, families, and other stakeholders to ensure a cohesive approach to patient care.

• Maintain a proactive schedule of appointments and home visits to support ongoing patient engagement and treatment progression.

• Plan and coordinate home visits and in-home assessments that contribute to a clearer understanding of patients' living environments and challenges.

• Ensure that the executed treatment plans are effective, monitoring and adjusting them as necessary in conjunction with interdisciplinary team members.

• Participate actively in Interdisciplinary Team Meetings to contribute insights and collaborate on care strategies for individuals.

• Document patient progress through monthly and quarterly notes, maintaining comprehensive records to support continuous care.
\n
• Provide mentorship and training for staff, ensuring compliance with treatment protocols and enhancing the team’s skills.
\n
• Offer follow-up sessions that include interviews and home visits to ascertain the ongoing requirements of individuals in care.

• Serve as a liaison to the public and the mental health community, fostering collaborative networks to achieve shared goals.

• Assist in the development and improvement of training materials and programs that enhance patient care delivery.

Qualifications\n
