Key Responsibilities\n

• Conduct comprehensive assessments and evaluations of patients’ psychological and social needs.

• Collaborate with an interdisciplinary team to develop and implement individualized care plans that address patients’ needs and family dynamics.

• Participate actively in the Individualized Family Service Plan (IFSP) process to ensure seamless and effective service delivery.
\n
• Develop and sustain rapport with patients and their families, providing ongoing support and education on coping strategies related to terminal illness.

• Utilize evidence-based practices and Natural Learning Environment Practices to enhance the experience of patients and families.
\n
• Monitor and document patient progress according to care protocols and regulatory requirements.
\n
• Advise families on resources available to them, equipping them with tools to support their loved ones effectively.
\n
• Ensure compliance with all federal and state regulations related to hospice care and social work practices.
\n
• Maintain confidentiality and respect for patient rights in all interactions.

Patient Population / Client Focus\n

You will be primarily working with patients within the hospice care setting, focusing on those with life-limiting conditions and their families. Your goal will be to provide psychotherapeutic support, facilitate discussions about end-of-life wishes, and assist families in navigating grief and loss. This role is specifically designed to meet the unique emotional and psychosocial needs of both children with serious health issues and their families, fostering an environment of compassion through challenging transitions.

Qualifications\n

• Master's Degree or Doctorate in Social Work.

• Current licensure as a Licensed Clinical Social Worker (LCSW) issued by the Department of Public Health.

Compensation and Benefits\n
