In-Home Health NP Contractor | Flexible Schedule & Perks

Signify Health

Signify Health, part of CVS Health, is hiring board-certified nurse practitioner contractors to conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Responsibilities include in-home visits to assess overall health, reviewing medications and history, and performing physical evaluations in the member's home using a pre-populated form.
