RN Case Manager - Home Health

We’re hiring an experienced Home Health RN Area Manager to lead clinical and operational teams while ensuring high-quality patient care and strong business performance. Key Responsibilities Lead, mentor, and develop clinical and administrative teams Oversee patient care, referrals, staffing, and care planning Ensure compliance with state, federal, Medicare, and company regulations Manage operational, financial, and quality performance Drive quality improvement initiatives Support business development and community partnerships Collaborate with executive leadership to achieve organizational goals Qualifications Current Minnesota RN license (or Minnesota-licensed Physical Therapist, as applicable) 5+ years of progressive healthcare experience , including leadership/management experience Home Health experience strongly preferred Excellent leadership, communication, and clinical decision-making skills Valid driver’s license and reliable transportation Ability to travel within the assigned service area CPR certification preferred Schedule Full-time Monday–Friday In-office position Participation in an on-call rotation Competitive Salary + Performance Bonus Our client is committed to Honoring People and is guided by Integrity, Compassion, Curiosity, and Helpfulness . #J-18808-Ljbffr
