Patient Flow Co-ordinator

Be at the heart of transforming patient care as a Patient Flow Coordinator (PFC) within the Transfer of Care Hub. In this fast-paced and rewarding role, you'll play a critical part in helping patients leave hospital safely, sooner, and with the right support in place. Working alongside a dynamic multidisciplinary team including nurses, social workers, and community partners you'll help drive seamless, person‑centred discharge planning. You'll connect the dots across acute and community services, gathering key information, coordinating care, and ensuring every patient's journey out of hospital is smooth, safe, and tailored to their needs. As a trusted link between wards, services, patients, and families, you'll combine compassion with coordination, supporting individuals through what can be a stressful time, while keeping patient flow moving across the wider health and care system. Your work will have a real impact, reduce delays, and ensure hospital beds are available for those who need them most. This is a role where no two days are the same.
