Key Responsibilities\n

• Conduct and document thorough bio-psychosocial assessments that address mental health, substance abuse, and medical needs for patients in the primary care setting.

• Deliver tailored behavioral health services, which encompass individual and group counseling, crisis intervention, intake interviews, treatment planning, and diagnostic evaluations.

• Provide social work liaison services, guiding clients to necessary resources within the community, including local mental health services and medication assistance programs.

• Collaborate with Quality Improvement teams to design and implement effective data collection processes that align with best practices for patient outcomes.

• Ensure timely documentation and updating of patient records, including progress notes and treatment plans, to maintain compliance with institutional policies and regulatory standards.

• Participate actively in team meetings to discuss patient cases, share insights, and strategize on approaches to enhance patient care.

Qualifications\n
