What You'll Do

• Provide care in nursing and long-term care facilities
• Manage chronic conditions and perform full assessments
• Close care gaps (HEDIS and STAR measures)
• Lead care planning with facility teams
• Support and educate patients' families

Must-Haves

• Active, unrestricted APN license in Michigan
• Board certified and Medicare credentialed
• 6+ months in skilled nursing or long-term care
• Solid with digital tools and team-based care
• Able to work onsite in Saginaw, MI

Perks & Pay

• Pay: $123,000-$135,000 base + corporate bonus
• Insurance: 90% medical/dental/vision (70% for dependents)
