Key Responsibilities\n

• Engage in thorough communication with physicians and other healthcare professionals to relay critical patient progress, needs, and potential concerns, ensuring that every client receives coordinated care.

• Maintain meticulous records, including clinical notes and progress documentation, adhering to the highest standards of confidentiality and organizational policies.

• Collaborate with a multidisciplinary healthcare team to develop and execute personalized treatment plans, including effective discharge strategies based on detailed psychosocial assessments.

• Provide guidance and crisis intervention for patients, employing evidence-based practices to address both emotional and behavioral challenges.

• Assist clients in navigating discharge planning, acting as a facilitator in coordinating access to essential services within the broader healthcare system.

• Consult with families and support systems to address treatment goals and ensure seamless discharge planning.

Patient Population / Client Focus\n

The clients at Clarity Wellness Center encompass a broad spectrum of backgrounds and mental health needs, including but not limited to anxiety, depression, relationship issues, and trauma-related conditions. You will have the opportunity to work with individuals, couples, and families, allowing you to utilize a variety of therapeutic modalities to cater to each person’s unique circumstances.

Qualifications\n

• Must hold a Master’s degree in Marriage and Family Therapy and possess a valid LMFT license.

• A minimum of 3-4 years of experience in an outpatient treatment setting is required, providing you with a robust foundation to support our clients effectively.

• Strong proficiency in computer skills, including experience with electronic medical records and telehealth platforms, as well as familiarity with MS Office applications.

• Demonstrated ability to work independently with a high degree of organization and self-direction.

Preferred Experience\n

Compensation and Benefits\n
