Key Responsibilities\n

• Conduct comprehensive mental health assessments and evaluations that consider the interplay between psychiatric and medical conditions impacting patient health.

• Design and implement personalized treatment plans that effectively integrate behavioral and physical health strategies.

• Facilitate both individual and group therapy sessions that are evidence-based and tailored to meet the diverse needs of our patients.

• Collaborate closely with medical providers and interdisciplinary teams to enhance treatment outcomes and ensure cohesive care delivery.

• Provide crisis intervention as necessary, responding to mental health emergencies with professionalism and empathy.

• Educate clients on the relationship between their mental and physical health, guiding them in managing their conditions more effectively.

• Maintain thorough and confidential patient documentation in compliance with all legal and ethical regulations.

• Participate in regular team meetings and case conferences to discuss patient progress and care plans, contributing to an agile and responsive care environment.

Patient Population / Client Focus\n

The primary focus of this role will be on individuals with co-occurring medical conditions and complex mental health needs. Qualifications\n

• Master’s or doctoral degree in Social Work, Mental Health Counseling, Psychology, Marriage and Family Therapy, or a related field from an accredited institution.

• Current licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Psychoanalyst, Psychologist, or Licensed Marriage and Family Therapist (LMFT) in New York State.

• Demonstrated experience working with patients who have co-occurring medical conditions and understanding their unique challenges.
\n
• Exceptional communication and interpersonal skills, allowing for effective rapport-building with patients and collaboration with healthcare teammates.

• Proven ability to maintain professional boundaries while ensuring patient confidentiality and adhering to ethical guidelines.

• Proficiency in utilizing electronic health record (EHR) systems and other digital tools to enhance patient care.

Compensation and Benefits\n
