Key Responsibilities\n

• Conduct comprehensive mental health assessments, evaluating the interplay between psychological and medical conditions to inform treatment planning.

• Develop and implement individualized integrated treatment plans that address both mental health needs and any co-occurring medical conditions.

• Provide individual and group therapy sessions utilizing evidence-based practices, ensuring approaches are personalized to each patient's circumstances.

• Collaborate with medical providers to ensure coordinated care, sharing updates and insights on patient progress to optimize treatment efficacy.

• Deliver crisis intervention services promptly and compassionately during mental health emergencies, helping to stabilize patients and connect them to ongoing support.

• Educate patients about the connection between mental health and physical well-being, empowering them with skills to manage their conditions effectively.

• Maintain meticulous and confidential health records in line with legal and ethical standards, utilizing electronic health record systems efficiently.

• Participate actively in team meetings and case conferences to ensure a cohesive and integrated approach to care.

• Stay updated on advances in mental health care and integrated treatment methodologies to continually refine skills and knowledge.

Patient Population / Client Focus\n

Our practice primarily serves veterans and their families, many of whom may be experiencing complex mental health challenges alongside physical health issues. This role requires a compassionate and empathetic approach to care, recognizing the unique backgrounds and needs of our patients. Our counselors will navigate challenges ranging from PTSD and anxiety to depression and substance use disorders, always striving to uplift our patients and foster resilience.

Qualifications\n

• A Master’s or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as an LMHC, LCSW, LCSW-R, Licensed Psychoanalyst, Psychologist, or LMFT within the state of New York.
\n
• Strong clinical knowledge and experience addressing the mental health needs of individuals with co-occurring medical conditions.

• Exceptional interpersonal and communication skills, with the ability to create strong rapport with patients.

• Proficiency in electronic health records and other related technologies.

• A collaborative spirit for working within an interdisciplinary team to achieve integrated care outcomes.

Preferred Experience\n

• A minimum of 2 years of experience in a clinical counseling setting, ideally focused on the veteran population or individuals with complex medical conditions.

Compensation and Benefits\n
