Key Responsibilities\n

• Conduct comprehensive assessments to evaluate the mental health needs of patients, ensuring tailored interventions.

• Maintain ongoing communication with supervisors, caregivers, and other relevant stakeholders about patient progress, ensuring continuity of care.

• Develop and manage a tentative schedule for appointments, home visits, and follow-up activities to monitor client development.

• Coordinate and conduct home visits as required, implementing data collection strategies like chart reviews and observational studies to facilitate care planning.

• Ensure effective execution of individualized treatment plans, adapting them as necessary to reflect clients' evolving needs.

• Actively participate in interdisciplinary team meetings, contributing insights on client care and developing holistic treatment approaches.

• Document monthly and quarterly progress notes comprehensively to track improvements and address any emerging issues.

• Coordinate comprehensive training programs, ensuring staff's competency in program implementation and documentation standards.

• Facilitate follow-up visits and interviews to provide sustained support, enhancing long-term outcomes for individuals.

• Serve as a liaison between the organization and the public, fostering relationships within the mental health community to support development initiatives.

Qualifications\n

Compensation and Benefits\n
