Key Responsibilities\n

• Conduct thorough assessments to evaluate and address the unique needs of each patient, ensuring personalized and effective treatment plans.

• Maintain open lines of communication with supervisors, colleagues, families, and other stakeholders to discuss patient progress and therapeutic adjustments.

• Provide supervisors with a calendar that reflects your appointments, home visits, and any necessary observations, enabling structured monitoring of case advancements.

• Plan and execute home visits for in-home assessments, incorporating chart reviews and data analysis to ensure a comprehensive understanding of individual patient needs.

• Oversee the execution of treatment plans, ensuring that all interventions are effective and aligned with best practices in mental health care.

• Engage in Interdisciplinary Team Meetings to collaborate on patient care and share insights that enhance treatment outcomes.

• Document monthly and quarterly progress notes that capture the evolution of clients’ conditions and treatment responses.

• Organize and facilitate training sessions for staff members, ensuring proficiency in the implementation of treatment strategies and documentation processes.

Qualifications\n
