Key Responsibilities\n

• Conduct thorough assessments to identify the social, emotional, and psychological needs of hospice patients and their families, ensuring each plan is tailor-made to suit individual circumstances.

• Provide empathetic counseling and support to individuals and families navigating end-of-life issues, helping them cope with grief, loss, and adjustment.

• Facilitate both individual and group counseling sessions utilizing evidence-based therapeutic methods, ensuring that clients feel heard and validated.

• Develop and implement individualized service plans that align with clients’ goals and therapeutic needs, fostering an environment of empowerment and dignity.

• Advocate for the rights of clients and ensure they have access to resources and support necessary for maintaining quality of life during this challenging time.

Patient Population / Client Focus\n

This role focuses specifically on hospice patients and their families, facing one of life’s most profound challenges. Qualifications\n

• A Master’s degree in Social Work from an accredited university or program.
