Location 3026 54th St #314,Lubbock, TX, 79424,United States Employee Type Full Time Contact information Name Dylan Douglas Description Clinical Manager – Home Health Full-Time The Clinical Manager is responsible for overseeing the coordination and delivery of high-quality home health services for Medicare and non-Medicare patients. This role ensures clinical care is delivered in accordance with agency standards, state and federal regulations, and industry best practices. The Clinical Manager provides clinical leadership, supports staff, monitors quality outcomes, and serves as a key liaison between patients, caregivers, physicians, referral sources, and the interdisciplinary care team. Clinical Leadership Oversee the daily clinical operations of the home health team. Ensure patient care is delivered safely, efficiently, and in accordance with agency policies and clinical standards. Serve as a clinical resource for nurses, therapists, and other interdisciplinary team members. Foster a positive, collaborative work environment by modeling the agency's mission, values, and standards of care. Patient Care Coordination Coordinate the delivery of home health services from admission through discharge. Review and assign new referrals to ensure timely patient admissions. Participate in referral intake and documentation within the Electronic Health Record (EHR) as needed. Collaborate with physicians, referral sources, patients, caregivers, and staff to ensure continuity of care. Facilitate recertification and discharge planning discussions to support appropriate patient outcomes. Monitor patient progress and ensure timely escalation of care when changes in condition occur. Quality & Compliance Ensure compliance with Medicare Conditions of Participation, agency policies, and all applicable state and federal regulations. Maintain strict adherence to HIPAA and patient confidentiality requirements. Monitor documentation for accuracy, timeliness, and regulatory compliance. Participate in quality improvement initiatives, audits, and performance improvement activities. Identify opportunities to improve clinical processes and patient outcomes. Staff Support & Operations Provide guidance, coaching, and support to clinical staff. Assist with workflow management and patient assignment to maximize efficiency and quality of care. Support implementation and oversight of the agency's 45-day workflow process. Participate in interdisciplinary team meetings and case conferences. Promote accountability, teamwork, and exceptional customer service throughout the organization. Required Qualifications Current, active Registered Nurse (RN) license in the state of practice. Associate Degree in Nursing (ADN) required; Bachelor's Degree in Nursing (BSN) preferred. Minimum of one year of home health experience; leadership or supervisory experience preferred. Strong clinical assessment, critical thinking, and decision-making skills. Working knowledge of Medicare Conditions of Participation, home health regulations, and accreditation standards. Excellent communication, organizational, and time management skills. Ability to work independently while leading and supporting a multidisciplinary team. Valid driver's license, reliable transportation, and current automobile insurance that meets agency requirements. Knowledge, Skills & Abilities Strong leadership and interpersonal skills.
