Description

American Traveler is looking for a Home Health Physical Therapist with a minimum of 1 year of Home Health experience and required HCHB/Oasis proficiency.

Details

Home Health setting covering the Santa Cruz / Watsonville area
Patient population consists of geriatric home health patients
Average caseload of 5-7 patients per day
Schedule is Monday-Friday, 8:30am-5pm, with a 30 minute lunch
No on-call or weekend requirements
Uses HCHB EMR system
Coverage area is within a 50 mile radius from the hub/office
Mileage reimbursed at IRS standard rate between patient homes

Requirements

Minimum 1 year of recent Home Health experience required
Previous Home Health or Hospice experience required
Current BLS certification required
CA PT license required in hand (license required if applicable)
HCHB charting experience required
Oasis experience required
First-time travelers will not be considered for this position

Additional Information

Local candidates accepted at the travel rate
Main responsibilities include evaluation and treatment of geriatric patients in the home health setting
