Job Description

Flexible shifts available any day, Monday through Sunday. Design your own work schedule. Choose your weekly or bi-weekly hourly commitment.

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members in their homes
• Review and document patients medical histories, current health status, and medications
• Educate patients about their conditions and prescribed medications, ensuring understanding and compliance
• Identify any gaps in care and recommend steps for improved patient outcomes
• Note: No treatment planning or prescribing of medications is required

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Up-to-date CPR certification (BLS)
• National certification through either AANP or ANCC
• At least 6 months of current Nurse Practitioner experience preferred
• MSN degree from an accredited nursing program
