Experienced Nurse Practitioners are needed to perform comprehensive home health risk assessments for Medicare Advantage members across Nash County. Youll enjoy the flexibility of creating your own schedule, choosing shifts between Monday and Sunday that fit your lifestyle, with the option for minimum weekly or bi-weekly hourly commitments.

Schedule:

• Flexible shifts available Monday through Sunday
• Choose your own workdays and hours
• Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

• Complete in-home annual risk assessments for Medicare Advantage plan members, averaging 45 minutes per visit
• Review and evaluate previous and current medical histories, existing medical conditions, and medication lists
• Deliver clear education to members about their health conditions and prescribed medications
• Identify gaps in care and recommend steps toward resolution
• Visits may include annual assessments or follow-up post-discharge visits
• No prescribing medications or creating new treatment plans

Qualifications:

• Current Nurse Practitioner license in North Carolina
• Active CPR certification (BLS)
• Board certification through AANP or ANCC
• MSN degree from an accredited nursing school
• At least 6 months of recent experience as a Nurse Practitioner preferred

This per diem position compensates $80-$90 per completed evaluation. In addition, you'll receive a mileage stipend and payment for approved administrative tasks.
