Boston Health Care for the Homeless Program is seeking a full-time Medical Case Manager to join their multidisciplinary HIV team. This role, based in Boston, focuses on assisting individuals living with HIV and substance use disorders, ensuring equitable access to healthcare. The successful candidate will assess patients’ needs, develop care plans, and conduct outreach. A background in human services and a commitment to harm reduction are essential.
