Work Environment

The role is based onsite at our assisted living facility in Clay, New York, a community that offers comprehensive support to older adults, many of whom may experience chronic health issues, including cardiovascular disease. You will collaborate with a multi-disciplinary team consisting of physicians, nurses, and allied health professionals to ensure comprehensive care for each resident.

Key Responsibilities

• Conduct thorough assessments to diagnose and treat various cardiology conditions, including heart failure, arrhythmias, and hypertension.
• Develop, implement, and evaluate customized treatment plans in collaboration with our healthcare team based on the unique needs of each patient.
• Provide comprehensive patient education on managing cardiology conditions, emphasizing lifestyle changes and preventative care practices.
• Regularly monitor and record patients’ progress, making necessary adjustments to treatment plans as indicated by clinical outcomes.
• Utilize state-of-the-art diagnostic equipment and tools to enhance patient evaluations and care protocols.
• Engage with patients’ families and caregivers to ensure a holistic understanding of treatment and care strategies.
• Participate in quality improvement initiatives and stay informed about the latest advancements in cardiology to integrate evidence-based practices into patient care.

Patient Population / Client Focus

Your primary focus will be on geriatric patients residing in our assisted living facility, many of whom may present complex medical histories and require careful and thorough management of their cardiovascular health. Your role will be essential in ensuring that their unique needs are met with compassion and specialized care.

Qualifications

• A Master of Science in Nursing (MSN) degree, with a concentration in cardiology or completion of a Family Nurse Practitioner program that includes robust cardiology coursework.
• A valid Cardiology Certified Nurse Practitioner (CCNP) credential from the Cardiology Nursing Certification Board (CNBC).
• A minimum of two years of clinical experience in cardiology or a closely related field as a Nurse Practitioner is required.
• Excellent communication, clinical reasoning, and organizational skills to effectively manage patient care and collaborate within a team.
• Proven ability to thrive in a team-oriented, fast-paced environment while demonstrating compassion and professional integrity.

Preferred Experience

• Experience working specifically in assisted living or geriatric care settings is highly preferred.

Compensation and Benefits
