Work Environment

In this role, you will work primarily onsite, visiting patients in their homes and in various community settings, including skilled nursing and assisted living facilities. Key Responsibilities

• Develop detailed, effective treatment plans tailored to individual patient needs, which may include seniors in assisted living or skilled nursing facilities.
• Order and interpret lab tests and diagnostic studies, adjusting treatment plans as necessary to enhance patient health outcomes.
• Maintain comprehensive documentation in our EMR system to ensure compliance with healthcare regulations and to facilitate seamless communication with care teams.
• Collaborate closely with local operations and clinical leadership to achieve strategic goals, enhancing service quality and operational efficiency.
• Engage in strategic initiatives to promote value-based care and improve patient satisfaction and retention rates.
• Provide continuous education to patients and families regarding health management, advanced care planning, and living healthier lifestyles.
• Conduct monthly, quarterly, and annual assessments in line with regulatory requirements, including acute visits as needed.
• Participate in family conferences to discuss care plans and provide necessary education on health issues.
• Evaluate and respond to pharmacy recommendations promptly, ensuring safe medication management for patients.
• Be physically present at assigned facilities during work hours to provide necessary in-person coverage for patient care needs.

Patient Population / Client Focus

Your primary focus will be on seniors, individuals in assisted living communities, and low-income patients, all of whom require varied levels of medical attention. You will address both acute and chronic conditions, reinforcing our commitment to delivering comprehensive health services tailored to underserved populations.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or equivalent graduate degree from an accredited institution.
• Current, active national certification from AANP or ANCC.
• Valid and unrestricted state nurse practitioner license.
• DEA registration for prescribing controlled substances.
• Authorization to work in the United States.
• Strong interpersonal and communication skills, with a passion for helping seniors and challenging populations.

Preferred Experience

• Experience in a concierge medicine or house calls setting is highly desirable.
• Previous work with seniors, particularly in skilled nursing or assisted living environments, is advantageous.
• Familiarity with EMR systems and telemedicine tools to enhance patient care efficiency.
• Ability to adapt to a flexible work schedule to meet patient needs and service demands.

Compensation and Benefits
