In this full-time, onsite position located in Clay, New York, you will play a crucial role in delivering primary and specialty medical services to our valued patients. Work Environment

Our clinic operates Monday to Friday from 9 AM to 5 PM, providing you with a structured schedule that prioritizes work-life balance. Key Responsibilities

• Conduct thorough health assessments and physical examinations to establish accurate diagnoses and develop individualized treatment plans tailored to each patient’s health journey.
• Utilize your clinical expertise to order and interpret diagnostic tests, prescribe medications, and perform therapeutic procedures, ensuring each patient receives comprehensive and holistic care.
• Engage in patient education, providing essential information and counseling on health promotion, disease prevention, and chronic disease management, empowering patients to take control of their health.
• Collaborate with interdisciplinary team members to effectively coordinate care, facilitate referrals, and ensure continuity of care, while actively contributing to quality improvement efforts within the practice.
• Maintain meticulous medical records, ensuring accuracy and confidentiality, in compliance with healthcare regulations and organizational best practices.

Patient Population / Client Focus

Your practice will focus on a diverse patient population ranging from those requiring primary care to those needing specialized medical attention. Given the concierge model, patients will appreciate the personalized service and the time you can dedicate to addressing their concerns. This role also supports patients in managing chronic conditions effectively, emphasizing preventive care and education.

Qualifications

• Master's or Doctoral degree in Nursing from an accredited program.
• Current licensure as a Nurse Practitioner in New York.
• Minimum of 2 years of clinical experience as an NP, demonstrating a solid foundation in patient care.
• Certification in a recognized Nurse Practitioner specialty area preferred.
• Exceptional clinical assessment and diagnostic skills, combined with an ability to make independent judgments.
• Outstanding communication and interpersonal skills, with a proven ability to build rapport with patients and their families.
• Proficiency in working collaboratively in a fast-paced healthcare environment geared towards concierge medicine.

Preferred Experience

• Experience in concierge medicine or home-based healthcare is highly desirable.
• Familiarity with electronic health record (EHR) systems and modern telehealth platforms.
• Demonstrated ability to navigate complex clinical scenarios and develop patient-centric solutions.

Compensation and Benefits

We offer a competitive salary range of USD 150,000 – 160,000 annually, commensurate with experience.
