Key Responsibilities

As a Nurse Practitioner specializing in Internal Medicine, your daily responsibilities will include:

• Conducting thorough initial patient histories and physical examinations to establish an accurate understanding of patient health needs.
• Developing, documenting, and communicating individualized plans of care while coordinating with other healthcare providers to optimize treatment approaches.
• Performing daily patient rounds to monitor progress, making necessary adjustments to treatment plans based on patient responses.
• Prescribing medications, ordering laboratory tests, and requesting diagnostic services as necessary to facilitate comprehensive care.
• Analyzing laboratory results and test outcomes, and conferring with other medical professionals to discuss findings and coordinate patient care efforts efficiently.
• Preparing and documenting discharge summaries, ensuring that patients are well-informed of their post-discharge requirements, including referrals and prescriptions.
• Engaging in ongoing research and performance improvement activities that contribute to the organization’s quality initiatives.

Patient Population / Client Focus

This role primarily serves adult patients managing chronic illnesses, acute health challenges, and complex medical needs. You will engage with a diverse patient population, including those requiring specialized home care due to mobility challenges, chronic conditions, or recovery from surgery. Qualifications

A successful candidate for this position will have:

• A Master’s Degree in a Nurse Practitioner Program, with National Board Certification in Internal Medicine or a related specialty.
• A valid Nurse Practitioner license in New York State, along with a current Registered Professional Nurse license.
• A collaborative practice agreement with designated protocols or a collaborative relationship attestation in compliance with the Nurse Practitioner Modernization Act 2015.
• Current Basic Life Support (BLS) certification.

Preferred Experience

While the following are not mandatory, they are highly preferred:

• Experience in home healthcare settings, particularly in managing complex patient needs.
• Background in performance improvement, patient education, and community health services.

Compensation and Benefits

This Nurse Practitioner role offers a competitive yearly salary ranging from $150,000 to $160,000, commensurate with experience and qualifications.
