Work Environment

Our clinic operates primarily during standard business hours, from 9:00 a.m. to 6:00 p.m., and may include occasional evening clinics until 8:00 p.m. This position involves approximately 40 hours of work per week, with a schedule that may require participation in community outreach events or health fairs during weekends.

Key Responsibilities

• Conduct thorough health assessments and screenings, analyzing both physical and mental health aspects to develop comprehensive care plans.
• Diagnose and manage a full spectrum of acute and chronic medical conditions, focusing on preventive care, health promotion, and patient education.
• Collaborate with community organizations to design and implement health education programs targeting specific health issues faced by the community.
• Provide culturally sensitive care by understanding and respecting the diverse backgrounds of patients, ensuring equitable treatment and advocacy for their healthcare rights.
• Coordinate referrals and follow-up care to guarantee continuity of treatment and track patient progress.
• Meticulously maintain detailed medical records that comply with healthcare regulations and standards.
• Engage actively in community outreach initiatives and health fairs, conducting health screenings and promoting awareness of health services.
• Stay informed about community health trends, evidence-based practices, and relevant policies to enhance care delivery.

Patient Population / Client Focus

The Nurse Practitioner will serve a diverse patient population, including families and individuals from various cultural backgrounds in Clay, NY. Qualifications

• A Master's or Doctoral degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• A current, unrestricted Nurse Practitioner (APRN) license in New York State.
• DEA# and NPI# required.

Preferred Experience

• A minimum of 3 years of clinical experience in primary care or family medicine, with a strong foundation in community health or public health settings.

Compensation and Benefits

We offer a competitive salary range of USD 120,000 – 130,000 per year, commensurate with experience and qualifications.
