Work Environment

The position operates in an onsite environment, allowing for intimate interactions with patients during house calls. You will work from 9:00 a.m. to 6:00 p.m., with occasional evening clinics that extend until 8:00 p.m. Participation in community outreach activities and health fairs on weekends may also be required. The average workweek encompasses approximately 40 hours, ensuring a balance between professional dedication and personal time.

Key Responsibilities

• Perform comprehensive health assessments and plan appropriate interventions for patients, ensuring thorough understanding of their medical history and lifestyle.
• Diagnose and manage both acute and chronic health conditions, providing timely preventive care and tailored health promotion strategies.
• Collaborate with local community organizations to design and implement impactful health education programs aimed at improving public knowledge and participation in health initiatives.
• Advocate for patient rights and equitable access to healthcare services, ensuring culturally sensitive care is prioritized throughout your practice.
• Coordinate patient referrals and follow-up care, working closely with specialists and other healthcare providers to maintain continuity in patient treatment.
• Ensure accurate and up-to-date patient documentation in compliance with healthcare regulations, utilizing electronic health records and maintaining confidentiality.
• Engage in community outreach events and screenings, enhancing public health awareness and promoting health services offered by Little Vista.
• Stay informed about trends in community health and incorporate evidence-based practices into your approach to patient care.

You will serve a diverse patient population, including young families, elderly individuals, and those facing socio-economic challenges in the Kirkville area. By providing house call services, you address the complexities and barriers that many patients encounter while seeking medical attention. Your role is pivotal in ensuring all individuals receive the care they need in a comfortable and familiar setting.

Qualifications

• Current certification as a Family Nurse Practitioner and an active nursing license in the state of New York.
• A Master’s degree in nursing from an accredited program.
• A minimum of three years of clinical experience, particularly in community health or public health settings.
• Demonstrated strong communication and interpersonal skills, enabling effective engagement with diverse populations.

Preferred Experience

Compensation and Benefits
