Position Overview

SLI Rehab & Physical Therapy is seeking a dedicated and compassionate Family Nurse Practitioner to join our team full-time in Manlius, New York. Work Environment

This position is based onsite at a school in Manlius, NY, providing a collaborative and vibrant environment where healthcare professionals work closely with educators and families. Your work schedule will typically run from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in community outreach events and health fairs during weekends may also be required. An average workweek will total approximately 40 hours, allowing for a balanced professional and personal life.

Key Responsibilities

• Conduct thorough health assessments and screenings for students and families to identify their physical, emotional, and behavioral healthcare needs.
• Diagnose, manage, and treat acute and chronic medical conditions, ensuring continuity of care and implementing preventive measures.
• Collaborate with local schools and community organizations to create and facilitate health education programs that inform and engage the community.
• Provide culturally sensitive and inclusive care, advocating for the rights and healthcare access of underserved populations.
• Coordinate patient referrals and follow-up care, ensuring all patients receive the necessary support and ongoing management of their conditions.
• Maintain accurate and detailed patient records in compliance with regulatory standards while ensuring confidentiality and security.
• Actively participate in community health initiatives and events to promote health awareness and provide screenings, reinforcing our commitment to community health.
• Stay informed on current healthcare trends, evidence-based practices, and relevant public health policies to enhance service delivery.

Patient Population / Client Focus

This position primarily focuses on children, adolescents, and their families within the school system and the broader community of Manlius. You will work with diverse populations, ensuring all individuals receive the tailored care they require, with an emphasis on preventive health and education. Your role will significantly impact improving health outcomes, particularly for vulnerable groups facing barriers to access.

Qualifications

• Current certification as a Nurse Practitioner.
• A valid state nursing license to practice in New York.
• A Bachelor’s degree in nursing and a Master’s degree from an accredited program.
• Minimum of 3-5 years of clinical experience, particularly in community health or public health settings.

Preferred Experience

• Experience working in school-based health programs or similar public health initiatives.
• Demonstrated ability to work with a diverse population, including underrepresented communities.
• Proficiency in electronic medical record (EMR) systems to maintain accurate healthcare documentation.
• Experience in developing and implementing health promotion and education programs.

Compensation and Benefits
