Health Partners Wellness Solution, LLC is seeking a dedicated and compassionate Senior Nurse Practitioner (NP) specializing in geriatric care to join our onsite team at our assisted living facility in Manlius, New York. Work Environment

This role will be primarily onsite within our assisted living facility in Manlius, New York. As a Nurse Practitioner, you will work closely with a multidisciplinary team, which includes physicians, nurses, pharmacists, and social workers, to provide holistic care. Key Responsibilities

• Collaborate with local and regional operations along with clinical leadership to meet healthcare delivery goals and improve patient outcomes.
• Support and implement strategic initiatives focused on value-based care designed to enhance the quality and efficiency of services provided to our senior residents.
• Deliver exemplary patient experiences that foster high levels of satisfaction and retention, ensuring residents feel valued and cared for.
• Practice within the ethical standards and scope determined by your professional board certification and applicable regulations.
• Maintain compliance with all state and federal regulations, ensuring all practices adhere to current healthcare laws.
• Engage collaboratively with Primary Care Physicians (PCPs) and interdisciplinary care teams to develop and adapt care plans that meet the needs of residents.
• Coordinate care journeys by collaborating with specialists and utilizing telemedicine solutions when applicable, ensuring timely access to necessary services.
• Conduct routine assessments and manage both chronic and acute medical conditions while promptly addressing changes in patients' health status.
• Provide health education to residents and their families, empowering them to make informed health decisions.
• Document all patient interactions comprehensively in our electronic medical records (EMR) system to maintain accurate and accessible health data.
• Oversee necessary regulatory visits, including monthly, quarterly, and annual evaluations, to adhere to healthcare standards.
• Participate in family meetings, facilitating advanced care planning discussions and educational sessions on available health services.
• Address any pharmacy recommendations, collaborating with pharmacists to optimize medication management for residents.

Patient Population / Client Focus

This position primarily focuses on the health and wellbeing of elderly residents in an assisted living environment. As a Nurse Practitioner, you will be responsible for addressing the unique healthcare needs of seniors, many of whom may have complex medical conditions or require assistance with daily living activities. Your expertise in geriatric care will be central to implementing individualized treatment plans that improve health outcomes and overall quality of life.

Qualifications

• A Bachelor’s Degree in Nursing; a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) is preferred.
• National certification from either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• An active and unrestricted state nursing license in New York.
• A valid Drug Enforcement Administration (DEA) license.
• US work authorization.
• A genuine passion for providing healthcare services to senior and low-income populations, helping contribute to a healthier community.

Preferred Experience

• Prior experience working in geriatric care or assisted living settings is highly desirable.
