Work Environment

This position is primarily onsite at an assisted living facility, where you will engage closely with residents who often have complex health needs. You will work in a supportive and friendly environment, emphasizing teamwork and collaborative problem-solving. With a scheduled workweek of approximately 40 hours, your hours will typically be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Some weekends may involve participation in community health fairs or outreach events.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, tailoring care plans to meet diverse healthcare needs.
• Diagnose and manage both acute and chronic healthcare conditions, focusing on preventive care and health promotion strategies.
• Collaborate with community organizations to develop impactful health education programs aimed at improving health literacy and outcomes.
• Provide culturally sensitive and patient-centered care, while advocating for patients' rights and equitable access to healthcare services.
• Coordinate patient referrals and ensure follow-up care to maintain continuity and quality of care.
• Document all patient interactions and medical records meticulously, adhering to regulatory standards and best practices.

Qualifications

• Current certification as a Family Nurse Practitioner.
• Active nursing license in the state of New York.
• Master's degree in nursing from an accredited program.
• A minimum of three years of clinical experience, ideally in community health, family medicine, or public health settings.

Compensation and Benefits

The Family Nurse Practitioner position offers a competitive salary range of $180,000 to $200,000 per year, commensurate with your experience and qualifications.
