Work Environment

The position is primarily onsite at our outpatient facility in Kirkville, NY, a welcoming environment designed to prioritize patient comfort and care. Our work schedule generally spans Monday to Friday from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Additionally, participation in community outreach events during weekends may be required, emphasizing our commitment to health promotion beyond the office.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients of all ages, identifying their physical and mental health needs.
• Diagnose and manage both acute and chronic medical conditions, offering preventive care and promoting healthy lifestyle choices.
• Develop and implement tailored health education programs in partnership with community organizations, enhancing public awareness and engagement.
• Provide culturally sensitive care, advocating for the rights of patients and ensuring that their healthcare needs are met with respect and dignity.
• Coordinate referrals for specialized care and ensure effective follow-up, maintaining continuity of care for our patients.
• Keep accurate and comprehensive medical records in compliance with healthcare regulations, ensuring prompt communication within the care team.
• Engage actively in community outreach initiatives and health fairs, delivering health screenings and educational resources to underserved populations.
• Stay informed on the latest community health trends, evidence-based practices, and relevant healthcare policies to provide informed guidance and support to our patients.

Patient Population / Client Focus

In this role, you will primarily serve individuals and families from diverse backgrounds, many of whom face barriers to accessing healthcare. Your efforts will not only impact individual health outcomes but also contribute to broader community well-being. You will work with patients ranging from children to seniors, addressing a variety of health issues through personalized care plans and preventive strategies.

Qualifications

• Hold a current certification as a Nurse Practitioner, with an active state nursing license.
• Possess a Bachelor's degree in nursing from an accredited institution; a Master's degree is preferred.
• Demonstrate a minimum of 3 years of clinical experience, with a focus on community health or public health settings highly desirable.
• Exhibit strong interpersonal and communication skills, effectively engaging with diverse populations in a compassionate manner.

Preferred Experience

Compensation and Benefits
