Work Environment

This full-time NP position is based onsite within a school environment in Clay, New York, allowing you to engage directly with students, staff, and families. Key Responsibilities

• Order and interpret a variety of lab tests and diagnostic exams, and adjust treatment plans based on the results to ensure optimal health outcomes for every student.
• Maintain accurate and compliant documentation utilising our cutting-edge EMR (Electronic Medical Record) system, which supports our commitment to efficient patient care.
• Work closely with local and regional operational leaders to align clinical goals with organizational strategies, thereby enhancing service delivery.
• Support and participate in initiatives that aim at advancing value-based healthcare delivery within the school.
• Deliver an outstanding patient experience by fostering positive relationships that encourage high levels of satisfaction and retention.
• Practice in alignment with ethical guidelines and the Scope and Standards of Practice, ensuring compliance with all applicable state and federal laws.
• Frequently collaborate with primary care providers (PCPs) and an interdisciplinary team to coordinate care efficiently.
• Facilitate telemedicine consultations and coordinate care with specialists to enhance the healthcare experience.
• Conduct regulatory-required visits and respond promptly to acute medical needs as they arise in the school setting.
• Engage in family conferences to provide education on advanced care planning when necessary.
• Review and act on pharmacy recommendations to ensure quality medication management for residents.
• Ensure adequate face-to-face coverage within the school to address health needs effectively during working hours.
• Assess, diagnose, and manage chronic and acute conditions while offering necessary health education to patients.
• Develop comprehensive treatment plans, clearly communicating these plans to students, families, and other healthcare providers.

Patient Population / Client Focus

In this pivotal role, you will primarily focus on providing care to students within the school. This patient population includes a diverse demographic, ranging from low-income families to students with special health needs. Your expertise will be critical in addressing both physical health concerns and educational barriers caused by untreated health issues, thereby enabling students to succeed academically and socially.

Qualifications

• Active and unrestricted state license to practice as a Nurse Practitioner.
• Valid DEA license for prescribing medications.
• Legal authorization to work in the United States.
• Master of Science in Nursing, Doctor of Nursing Practice, or successful completion of an NP certificate program.
• National certification from either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Proven ability to adapt to flexible schedules and work environments.

Preferred Experience
