Work Environment

You will be working in an Assisted Living Facility, specifically designed to provide a comfortable and safe living environment for older adults. Key Responsibilities

• Conduct thorough assessments of patients' health status by performing health interviews and detailed physical examinations.
• Gather, update, and analyze comprehensive medical histories to inform treatment plans.
• Collaborate with physicians in developing short-term management plans and long-range care strategies for patients with chronic conditions.
• Manage patient treatments and interventions, ensuring continuity of care and adherence to established protocols.
• Educate residents and their families about health management, preventive care, and lifestyle changes to enhance health outcomes.
• Document all patient interactions, clinical findings, and care plans accurately in electronic health record systems to facilitate communication amongst the healthcare team.
• Participate in multidisciplinary team meetings to discuss patient progress, treatment plans, and any necessary adjustments.
• Assist in coordinating referrals and additional healthcare services to support comprehensive patient care.

Patient Population / Client Focus

You will primarily care for geriatric patients residing in our Assisted Living Facility, many of whom may have complex medical histories and require a strong focus on their internal medicine needs. Your ability to build rapport with patients and families will be essential in creating a trust-based environment where patients feel valued and understood.

Qualifications

• A Bachelor’s Degree in Nursing or a related field.
• Certification as a Nurse Practitioner or Physician Assistant, with a current license to practice in New York State.
• Completion of an accredited NP or PA program with a focus on internal medicine or geriatric care.

Preferred Experience

• At least 2 years of experience in an ambulatory or assisted living care setting, with a focus on internal medicine.
• Strong interpersonal and communication skills to facilitate effective interactions with patients, families, and colleagues.
