Work Environment

The role is based in our outpatient office, providing a supportive and collaborative environment where you will work alongside a skilled care team. This team typically includes a physician, other nurse practitioners or physician assistants, medical assistants, and care managers. You will also have the support of a dedicated medical scribe, allowing you to focus on patient care rather than administrative tasks. This structure enhances our ability to coordinate care effectively with specialists and hospitals in our extensive network.

Key Responsibilities

• Deliver exceptional primary care by seeing patients for routine follow-ups, acute illnesses, and preventative care visits.
• Conduct thorough annual wellness visits and health risk assessments, requiring a holistic approach and meticulous attention to documentation.
• Coordinate care across various providers, specialists, and community resources to ensure comprehensive treatment plans.
• Lead population health initiatives, such as ensuring timely screenings for conditions like breast cancer, to maximize health outcomes.
• Provide education to patients about their healthcare issues and treatment plans, fostering empowerment and self-management.
• Engage in phone triage and patient outreach to improve care access and continuity.
• Participate in promotional activities to enhance community engagement and awareness of our services.
• Conduct home visits as necessary to provide care for those who may be homebound.
• Report to the Medical Director and interact closely with operational leadership to maintain quality standards.

Patient Population / Client Focus

Our primary focus is on older adults who are typically on Medicare, many of whom have complex health needs requiring thorough management and preventative care strategies. You will play a pivotal role in helping our patients navigate their healthcare options, building trusting relationships that facilitate better understanding and adherence to their health plans.

Qualifications

• A Master's Degree in Nursing or completion of a Doctor of Nursing Practice program is required.
• National certification as a Nurse Practitioner in Family, Adult, or Adult-Gerontology Primary Care through AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license and DEA license are mandatory.
• Experience in primary care settings such as internal medicine, geriatrics, or family medicine is essential.
• Familiarity with HCC documentation, ICD-10 coding, and health risk assessments is advantageous.
• A collaborative mindset and a passion for working with a multidisciplinary team are essential traits.

Preferred Experience

• Bilingual skills in languages pertinent to our patient population can enhance healthcare delivery.
• A strong understanding of quality improvement processes in healthcare is desirable.

Compensation and Benefits

• Competitive salary range of $180,000 to $200,000 per year, commensurate with experience and expertise.
