Key Responsibilities

• Conduct thorough and comprehensive health assessments through physical examinations and patient interviews.
• Order, perform, and interpret diagnostic tests such as lab work and imaging studies to inform clinical decisions.
• Collaborate with our integrated care team to develop and implement individualized patient care plans tailored to unique health needs.
• Prescribe appropriate medications while adhering to established protocols and guidelines.
• Provide health education to patients and their families, empowering them with vital information on managing their health.
• Participate in regular team meetings and contribute to case discussions, ensuring seamless coordination of patient care.
• Maintain accurate and up-to-date electronic health records (EHR), documenting all patient interactions and care outcomes.

Patient Population / Client Focus

This role focuses on a diverse range of patients, including children, adults, and the elderly, within the concierge medicine model. By engaging directly with patients through house calls, you will have the opportunity to build meaningful relationships and provide comprehensive care in a familiar environment. Qualifications

• A valid and unrestricted Nurse Practitioner license in New York State.
• A minimum of 2 years of clinical experience as a Family Nurse Practitioner, preferably in a primary care or concierge setting.

• Exceptional clinical assessment, diagnostic, and treatment planning abilities.

Compensation and Benefits

The annual salary for this role ranges from $150,000 to $160,000, commensurate with experience and qualifications. Additionally, our comprehensive benefits package includes:
