Work Environment

This position is primarily onsite in the Kirkville area, focusing on home care. You will engage directly with patients in their own environments, providing you with unique insights into their lives and health challenges. The work schedule will typically span from 9:00 a.m. to 6:00 p.m., with flexibility for occasional evening clinics until 8:00 p.m. Participation in health fairs and community outreach events may also be required, especially on weekends, reflecting our organization's commitment to proactive community health engagement. You can expect an average commitment of around 40 hours per week.

Key Responsibilities

• Conduct thorough health assessments, including history taking and physical examinations, to determine the needs of individuals and families.
• Diagnose and manage both acute and chronic health conditions while focusing on preventive care and promoting health literacy.
• Develop, implement, and evaluate health education programs in direct collaboration with community partners to address local health trends effectively.
• Advocate for the rights and access of patients to healthcare, ensuring culturally sensitive approaches are employed in all interactions.
• Coordinate and facilitate referrals to specialists and ancillary services to provide a continuum of care, ensuring clear communication and follow-up.
• Maintain comprehensive, up-to-date medical records in compliance with established healthcare regulations and organizational policies.

Qualifications

Compensation and Benefits
