Key Responsibilities\n

As a Licensed Clinical Social Worker with Navigate Scape, your daily responsibilities will include:

• Collaboratively developing and executing evidence-based treatment plans tailored to each client's specific needs and goals.

• Setting appropriate milestones and benchmarks to assess client progress, adjusting treatment plans as necessary to ensure positive outcomes.

• Communicating vital updates to relevant parties, including clients’ families and other healthcare professionals, to foster a comprehensive treatment approach.

• Facilitating referrals to additional healthcare resources and support programs, ensuring clients have access to a full spectrum of care.

• Maintaining accurate, detailed client documentation in compliance with state privacy laws, and preparing regular reports on client outcomes.

• Conducting crisis assessments and implementing appropriate interventions to support clients during emergencies.
