Hospice RN Care Manager - Lead Clinical Excellence

Gentiva Hospice is seeking a Patient Care Manager to oversee clinical operations and coordinate care at our Oak Brook location. You will mentor staff, ensure compliant, high-quality hospice services, and lead visits scheduling with clinical judgment.
