At Choice Health at Home, exceptional patient care starts with exceptional coordination. We're looking for a detail-oriented, organized, and compassionate Patient Services Coordinator (PSC) to help ensure our patients receive timely, seamless home health services.

What You'll Do

As a Patient Services Coordinator, you'll play a vital role in keeping our clinical team organized and our patients cared for by:

• Coordinating and scheduling home health visits for clinicians
• Prioritizing new patient admissions to ensure timely starts of care
• Communicating with nurses, therapists, patients, physicians, and referral partners
• Managing scheduling workflows within our electronic medical record system
• Providing outstanding customer service to patients and healthcare professionals
• Assisting with medical records functions through cross-training as a Medical Records Specialist
• Supporting quality improvement initiatives and agency operations
• Helping ensure every patient receives the highest level of coordinated care

Clinical (LVN/LPN) candidates may also have the opportunity to perform patient visits as assigned.

What We're Looking For

• High school diploma or equivalent required
• LVN/LPN license preferred but not required
• One year of home health or hospice experience preferred
• Strong organizational, multitasking, and communication skills
• Experience working in a fast-paced healthcare environment is a plus
• Reliable transportation and valid driver's license
• CPR certification required only if performing patient visits
