Key Responsibilities\n

• Conduct thorough, compassionate assessments to identify clients' psychosocial needs and develop individualized treatment plans.

• Maintain accurate and confidential client records in accordance with HIPAA regulations, utilizing clinical software for documentation.

• Implement evidence-based interventions and participate in regular team meetings to discuss client progress and adjust care plans as needed.

• Provide bereavement support and counseling for family members during and after the patient's transition.

• Engage in ongoing professional development and stay current with best practices in social work, particularly in hospice and palliative care.

Patient Population / Client Focus\n

Your primary focus will be on patients who are experiencing life-limiting illnesses, along with their families. Understanding the complexities of grief, anxiety, and emotional distress is crucial as you provide support. Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• An active Licensed Clinical Social Worker (LCSW) license in the state of Georgia is required.
