This role operates remotely, allowing LCSWs the flexibility to work from the comfort of their homes while still being an integral part of a dedicated team of professionals. As part of a hospice center, our remote support staff collaborates closely to ensure every client receives personalized treatment. You will leverage teletherapy tools to maintain a therapeutic rapport with clients and their families, ensuring seamless care and communication.

Key Responsibilities\n

As an LCSW with Premire Counseling & Wellness Services, you will have the distinct responsibility of:

• Assessing the counseling needs of clients through initial consultations and ongoing evaluations.
\n
• Utilizing DSM criteria for substance abuse diagnoses to determine appropriate care levels based on ASAM guidelines.
\n
• Developing and implementing treatment plans that address substance use issues as part of comprehensive care.
\n
• Creating discharge and continuity of care plans to ensure smooth transitions to subsequent levels of care.
\n
• Maintaining detailed records of counseling sessions, assessments, and treatment progress in clinical documentation.
\n
• Effectively managing a caseload, including conducting regular follow-up and case management to track treatment efficacy.
\n
• Coordinating support services for clients and their families and keeping thorough records of these interactions.
\n
• Engaging with referral sources, including legal entities and educational institutions, to facilitate comprehensive support.
\n
• Addressing crisis situations through effective interventions and a multi-disciplinary team approach.

Patient Population / Client Focus\n

Your primary focus will be working with clients experiencing terminal illness and end-of-life issues. You will provide compassionate support and therapeutic services tailored to each client's unique circumstances. Your role will also extend to assisting families in dealing with emotional stress, grief, and loss, ensuring they have the coping mechanisms and resources needed during a difficult time.

Qualifications\n

To be considered for this position, you must possess:

• A Master's degree in Social Work from an accredited institution and a valid LCSW license in Texas.

Compensation and Benefits\n

We offer a competitive salary ranging from $70,000 to $100,000 annually, commensurate with experience. In addition, our comprehensive benefits package includes:
