Are you an experienced Social Worker with acute inpatient case management and discharge planning expertise ? We're seeking a highly skilled professional to join a fast-paced Level I Trauma and Academic Medical Center, helping patients transition safely from hospital to home or post-acute care settings.

Position Overview:

• Degree: Social Worker

• Specialty: Social Work Care Coordinator (Discharge Planning)

• Job Type: Travel

• Assignment Length: 13 Weeks

• Start Date: July 9, 2026

• Shift: 12-Hour Days (7:00 AM–7:00 PM)

• Schedule:

  • Every Wednesday and Thursday

  • Every other Friday, Saturday, and Sunday

• Guaranteed Hours: 36/48-hour rotating schedule every two weeks

• Location: Jackson, MS

• Work in a Level I Trauma and Academic Medical Center

Requirements:

• Active LMSW or LCSW license

• Active Mississippi Social Work license

• BLS Certification

• Minimum 2 years of acute inpatient medical case management experience

• At least 1 year of recent experience (within the last 3 years) in a 500+ bed Level I Trauma or Academic Medical Center

• Previous travel experience in a Level I facility within the last 3 years

• Recent acute inpatient discharge planning experience

• Proficiency with Epic EMR

• Strong computer and documentation skills

• Ability to work independently with minimal orientation
