Reilley Physical Therapy is a private outpatient mobile practice serving Tompkins and Cortland Counties and surrounding area that provides outpatient physical therapy and wellness/exercise services in the home of the patient. Reilley Physical Therapy is searching for a committed Physical Therapist with an interest in working with older adults across a wide-range of functional levels including medically complex adults, independent community dwelling older adults and senior athletes. The ideal candidate will act as the patient’s partner throughout the journey of restoring movement and ensuring that the patient will function at their personal best. You will maximize the quality of life and movement potential within the spheres of promotion, prevention, diagnosis, treatment or intervention, and rehabilitation.

Responsibilities

• Travel to patient's homes to provide PT services
• Identify and meet patients’ goals and needs
• Develop care plans using a variety of treatment techniques

• Create and implement effective discharge plans including fitness- and wellness-oriented programs tailored to patients’ specific needs
• Provide quality, personalized and evidence-based care and proven interventions
• Motivate patients during treatment in order to help them function optimally
• Promote clients’ healthy lifestyle by improving power, strength, flexibility, balance, and coordination
• Consult and practice with other health professionals
• Evaluate effects and monitor and communicate progress
• Document patient care services using the APTA's defensible documentation standards

Skills

• At least 2 years of work experience as a physical therapist, preferably in the outpatient and home care settings.
• Experience and interest in working with older adults in the outpatient and/or the home care setting
• Intensive education and clinical expertise
• Ability to manage patients with different types of personalities
• Current knowledge of evidence based practice
• Drive to continually learn and grow
• Professional behavior
• Excellent interpersonal communication skills
• Valid New York State physical therapy license
• Board Certification in any of the following preferred: Geriatric Physical Therapy, Cardiopulmonary Physical Therapy, Neurologic Specialist.
