Pulse Point Internal Medicine is actively seeking dedicated and compassionate Nurse Practitioners to join our team in Ithaca, New York. As part of a larger healthcare ecosystem, we engage in partnerships with elite specialists and hospitals, ensuring that our Nurse Practitioners can focus primarily on comprehensive primary care, care coordination, and enhancing population health.

Work Environment

This role will take place in our onsite hospital-based outpatient practices. You will work within a dynamic and collaborative healthcare team that includes a physician, other nurse practitioners, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. Key Responsibilities

• Deliver exceptional primary care services tailored to individual patient needs.
• Conduct thorough office visits addressing routine and acute health concerns, ensuring that each appointment is both comprehensive and supportive.
• Administer annual wellness visits and health risk assessments with meticulous attention to documentation, focusing on the holistic health of each patient.
• Coordinate care with various healthcare providers and specialists, as well as testing facilities and agencies, to create seamless transitions for patient referrals.
• Lead population health initiatives in coordination with the Care Team, ensuring adherence to evidence-based guidelines such as breast cancer screenings.
• Assist the healthcare team with phone triage and outreach to engage patients further in their health journey.
• Educate patients thoroughly on their health conditions, care plans, and treatment options, empowering them with knowledge to make informed decisions.
• Participate in promotional activities and community outreach to reinforce our presence and mission in Ithaca.
• Conduct home visits as necessary to provide care to those who may face barriers to accessing our clinic.
• Report directly to the Medical Director while working closely with operational leadership to continually improve care processes and outcomes.

Patient Population / Client Focus

This role focuses primarily on serving older adults enrolled in Medicare, requiring a nuanced understanding of their unique healthcare needs and challenges. Our mission is to provide personalized care that fosters these patients' health and well-being, allowing them to lead fulfilling lives while managing their healthcare proactively and compassionately.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner conferred by the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• An active, non-probationary Nurse Practitioner license in the state of New York.
• An active DEA license to prescribe medications.
• A minimum of two years of experience in primary care settings, such as internal medicine, geriatrics, or family medicine.
• Knowledge of Hierarchical Condition Categories (HCC) documentation and the International Classification of Diseases, Tenth Revision (ICD-10) coding.

Preferred Experience

• Experience with Health Risk Assessments (HRAs) and an understanding of population health management.
• A passion for collaborative teamwork and cross-functional partnerships.
• Bilingual proficiency in relevant languages, enhancing communication with diverse patient populations.

Compensation and Benefits

The annual salary range for this position is USD 180,000 to 200,000. This range is negotiable based on experience and expertise. Our comprehensive benefits package includes:
