Work Environment

The Nurse Practitioner will be engaged in both office-based patient evaluations and house calls, allowing for a flexible, patient-centered approach to healthcare delivery. Key Responsibilities

• Conduct comprehensive primary care assessments, including initial physical examinations and detailed patient history evaluations.
• Develop and implement individualized treatment plans based on patient assessments, lab results, and ongoing health evaluations.
• Order and interpret diagnostic tests, ensuring timely communication of results to patients and developing subsequent treatment options or referrals.
• Utilize and manage medical equipment effectively for patient examinations and treatment protocols, ensuring adherence to safety and quality standards.
• Provide education to patients and their families regarding treatment options, preventive measures, and lifestyle modifications necessary for managing cardiovascular health.
• Make appropriate referrals to specialists and coordinate with other healthcare providers to facilitate holistic patient care.
• Document all patient encounters, treatment plans, and medical histories accurately in the electronic health record (EHR) system.

Patient Population / Client Focus

The Nurse Practitioner will primarily work with individuals who have cardiovascular conditions or concerns, including but not limited to hypertension, heart disease, and arrhythmias. Our patient demographic includes adults from diverse backgrounds, many of whom come from underserved populations requiring enhanced accessibility to high-quality healthcare. Qualifications

• Current certification as a Nurse Practitioner in the state of New York.
• An active nursing license must be maintained without any disciplinary action.

Preferred Experience

Compensation and Benefits
