Work Environment

As part of our Cardiology department, you will be working directly with cardiac procedural patients in their home settings, allowing for individualized care that considers their specific health challenges and living situations. Key Responsibilities

• Conduct comprehensive health assessments and screenings to evaluate the physical, emotional, and social health of patients with cardiac issues.
• Diagnose and manage acute and chronic cardiovascular conditions, including the implementation of preventive care and health promotion initiatives.
• Collaborate with a multidisciplinary team, including cardiologists, nurses, and social workers, to devise tailored care plans that improve patient health outcomes.
• Engage with community organizations to advocate for health education programs aimed at promoting cardiovascular health awareness.
• Deliver culturally sensitive care and ensure advocacy for patients’ rights, improving their access to essential healthcare services.
• Maintain precise and compliant medical records, adapting to best practices and regulatory standards for documentation.
• Participate in community outreach and educational events to enhance public knowledge about cardiovascular health issues, providing essential screenings and consultations.
• Stay abreast of the latest trends in community health, evidence-based practices, and changes in relevant healthcare policies.

Patient Population / Client Focus

This role will particularly focus on patients with cardiovascular diseases, ranging from those with chronic conditions such as heart failure and coronary artery disease to patients recovering from cardiac procedures. You will be responsible for nurturing relationships that ensure these patients receive comprehensive care tailored to their unique circumstances and health journeys.

Qualifications

• A valid Nurse Practitioner credential.
• Current nursing licensure in New York State.
• A Master's degree in nursing from an accredited institution.
• At least two (2) years of clinical experience within a Cardiology setting.
• Exceptional communication and interpersonal skills, enabling you to work effectively with diverse populations.
• A strong commitment to issues of social justice and health equity, focusing on advocating for underrepresented communities.

Preferred Experience

Compensation and Benefits

We offer a competitive salary range of $150,000 to $160,000 annually, dependent on experience and qualifications.
