This position is primarily focused on providing exceptional outpatient care within the patient's home environment, allowing for personalized health management. The position offers a competitive salary range of USD 150,000 to 160,000 annually, commensurate with experience.

Work Environment

This Nurse Practitioner role will be conducted in the home care setting in Ithaca, New York, enabling you to provide tailored patient care in a familiar and supportive environment. You will experience flexibility in your work schedule, transitioning from an initial three-month training period of five 8-hour shifts per week, to a more adapted four 10-hour shifts schedule thereafter, effectively balancing work with life commitments.

Key Responsibilities

• Conduct detailed patient histories and thorough physical examinations, ensuring comprehensive understanding of health conditions.
• Perform and order diagnostic tests tailored to patient needs, critically analyzing results to plan appropriate treatments.
• Prescribe both pharmacological and non-pharmacological interventions in accordance with standardized treatment guidelines.
• Maintain meticulous patient records, documenting orders and progress notes that reflect ongoing treatment and patient status.
• Oversee the accuracy of patient medical records, ensuring compliance with healthcare regulations.
• Deliver continuity of care through follow-up health maintenance strategies as established in collaboration with supervising physicians.
• Facilitate effective communication between patients, their families, and the healthcare team, fostering a supportive care environment.

Patient Population / Client Focus

This role focuses on a diverse patient population with varying cardiovascular health needs, including those with chronic conditions and acute care requirements. Your ability to connect and communicate with patients from different backgrounds will crucially contribute to improving their understanding and management of heart health.

Qualifications

• A Master’s Degree in Nursing from an accredited institution.
• Active and valid Registered Nurse (RN) license in the State of New York.
• A minimum of three years of clinical experience in nursing, with a strong emphasis on cardiology preferred.
• Basic Life Support (BLS) Certification is required.
• Exceptional interpersonal and communication skills, adept at interacting with diverse patient populations.

Preferred Experience

• Experience working in a home healthcare setting is highly regarded.
• Knowledge of electronic health record (EHR) systems to streamline patient documentation.
