Work Environment

This position is based onsite, allowing NPs to engage closely with patients in their homes. Key Responsibilities

• Conduct thorough patient evaluations through home visits, assessing health status, identifying conditions, and determining treatment strategies.
• Develop personalized treatment plans based on best practices and individual patient circumstances, closely monitoring outcomes to ensure optimal health results.
• Prescribe medications and therapeutic interventions in accordance with established medical guidelines and state regulations, ensuring patient safety and efficacy.
• Perform minor surgical procedures as necessary and provide assistance in more complex surgical scenarios, adhering to established protocols.
• Educate patients and their families about disease prevention, health maintenance, and lifestyle changes necessary for improved health outcomes.
• Work proactively with interdisciplinary teams to support the facility’s quality assurance objectives and overall mission to provide outstanding healthcare services.

Patient Population / Client Focus

The Nurse Practitioner will serve a wide range of patients, including elderly individuals, those with chronic illnesses, and patients requiring post-surgical recovery care. Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of a Nurse Practitioner Certificate Program.

• An active, unrestricted state license to practice as a Nurse Practitioner in New York.

Compensation and Benefits

• A competitive salary range of $150,000 to $160,000 per year, commensurate with experience and patient caseload.
