Gundersen Health and Wellness Clinic is actively seeking a dedicated and skilled Nurse Practitioner (NP) to join our dynamic team within our Cardiology Clinic. The ideal candidate will be responsible for providing high-quality outpatient care as part of the Center for Advanced Cardiac Care (CACC) under the guidance of the Chief Nurse Practitioner and the oversight of the Medical Director. This full-time position offers an exciting opportunity to make a direct impact on patient care within a comprehensive cardiac care environment and will involve working directly onsite in Ithaca, New York.

Key Responsibilities

• Conduct thorough patient assessments by obtaining detailed medical histories and performing comprehensive physical examinations.
• Order and interpret necessary diagnostic tests to identify and manage both chronic and acute cardiac conditions.
• Develop and implement individualized, goal-directed care plans that incorporate health maintenance and nursing interventions.
• Collaborate with physicians to modify treatment plans as needed based on patient progress and feedback.
• Educate patients and their families about disease processes, treatment options, and effective care management strategies to promote wellness beyond the hospital environment.
• Coordinate and facilitate hospital admissions and discharges, ensuring seamless communication with the inpatient team and providing safe transfer of care.
• Engage in triaging daily patient inquiries, addressing concerns, and facilitating communication among healthcare providers.
• Document all patient interactions in the electronic medical records (EMR) system, ensuring accuracy and compliance with organizational standards.
• Participate in clinical research initiatives, applying the latest evidence to practice and continually enhancing care delivery.
• Maintain accountability for practice within the Nurse Practitioner’s scope of care, contributing to a safe and effective healthcare environment.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve patients with complex cardiac health issues, including individuals undergoing advanced therapies such as heart failure management, outpatient intravenous therapy, and mechanical circulatory support. You will play a vital role in the management of patients experiencing various stages of cardiac disease and contribute to improving their quality of life through comprehensive care strategies.

Qualifications

• Possession of a Master’s degree in Nursing, with specialization as a Nurse Practitioner trained in adult care.
• A current New York State Registered Professional Nurse license and Nurse Practitioner certification.
• Board certification in Nurse Practice and current certifications in BCLS, ACLS, DEA, and NPI.
• Proficiency in utilizing electronic medical records (EMR) systems and familiarity with software such as MS Office Suite.

Preferred Experience

• At least two years of experience in a clinical setting, preferably within cardiology or related disciplines.
• Experience managing complex patient populations such as those facing heart failure, pulmonary arterial hypertension, and those who have undergone heart transplant procedures.
• Knowledge of advanced therapeutic options and outpatient care techniques to support cardiac patients.

Compensation and Benefits

The Nurse Practitioner position offers a competitive salary ranging from $120,000 to $220,000 annually, based on experience and qualifications.
