Key Responsibilities\n

• Conduct thorough ongoing assessments and develop individualized treatment plans based on the unique needs and circumstances of each patient.

• Maintain open lines of communication with supervisors, direct care staff, family members, and other significant individuals involved in a patient's care to discuss progress and address any concerns.

• Create and share a tentative schedule of appointments, home visits, and observations to maintain accountability regarding patient progress.
\n
• Coordinate essential home visits to facilitate in-home observations, conduct chart reviews, and engage in staff interviews as part of the treatment process.
\n
• Ensure implementation of program plans is in accordance with best practices and effectively addresses patient needs.
\n
• Participate actively in Interdisciplinary Team Meetings to contribute to holistic care strategies and collaborate with other healthcare professionals.
\n
• Document monthly and quarterly progress notes to provide insight into patient progress and inform future care decisions.
\n
• Organize and conduct training sessions to enhance staff competencies in implementing treatment plans and documentation protocols.
\n
• Provide ongoing support through follow-up interviews, home visits, and observations to monitor ongoing patient needs and success.
\n
• Safeguard patient records and information, ensuring compliance with privacy regulations and facilitating easy access for authorized individuals.

• Act as a representative and liaison with the broader public and mental health community to promote development goals and foster partnerships.

• Assist in the development and refinement of training materials and programs aimed at enhancing the effectiveness of our services.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field from an accredited institution.

• Possession of an active and unrestricted Professional Counselor (LPC) licensure in the state of Texas.
